Terms and Conditions
The Monthly Training Contract (MTC) is in place for a minimum of six (6) months before any penalty free cancellations are enforced. During that six (6) month period there will be an automatic renewal for the MTC for each of the subsequent one month periods.

All must be done via email within at least 30 days in advance of the next billing date.

By agreeing to the terms below I acknowledge that I understand that I will be billed monthly for 6 months. If I cancel this agreement before the six month commitment expires, I understand that I will be charged an early cancellation fee equal to the total value of the sessions remaining in the contract.

As part of your MTC package, subject to the limitations on the use specified above, you will be entitled to receive the following:
6 month membership (minimum)
3 TRY PACKS 10% off for friends and family interested in taking on a personal trainer
1-2 virtual training session weekly (1hr each)
Early access to all new Tag Along content (NEW)
(24)hr cancellation policy
(24)hr unlimited online support
Fitness challenges and retreats
Agreement to Pay Recurring Fees: agrees to pay monthly recurring fees as follows:
Client authorizes monthly automatic credit card and/or checking account ACH charges. Alternatively, client may prepay six or twelve months in advance via any credit card, ACH, bank check or wire transfer. Payment in monthly increments by any means other than client's credit card or ACH is not permitted.

I agree to recurring (monthly) charge to my credit card, or automatic debit to my checking account each month for a period of 6 months. If I desire to cancel after the sixth month I need to do so in writing 30 days prior to my next billing date.
(b) I hereby certify that I am the holder of the credit card, or an authorized signer on the bank checking account detailed below.
(c) I understand that I will be notified if my credit card or checking account payment fails to authorize for any reason, and that a $39 late fee will be automatically applied if I do not provide a valid credit card or checking account ACH information within 3 calendar days of the original rejection date.
(d) I understand that my service will be put on hold if my account becomes more than 30 calendar days late.
(e) I understand that delinquency of payment and the subsequent hold placed on my account doesnt remove my responsibility of payment.
(f) I understand that if my account becomes delinquent and is subsequently placed on hold that I can have all holds removed after bringing the account to current.

CANCELLATION RIGHTS: You may cancel this agreement without any penalty within (3) days from the date listed above. Cancellation prior to the end of the commitment period above, you will be charged a cancellation fee equal to the total value of the sessions remaining on the contract.

FREEZING YOUR AUTO-PAY
To freeze your MTC auto pay, a completed request must be submitted in email at least 30 days in advance of your next billing date in order for your request to be effective for the following month.
Clients enrolled in the MTC auto renew program may elect to freeze their auto pay charges no more than twice per year according to the policy below:
You can freeze your auto pay schedule twice per year at any time for a fee of $35 per month. The freeze is activated at the start of the next billing cycle and can be requested via email in monthly increments for up to 6 months.
To request that your MTC be frozen you must simply email info@supremetraining.org and be certain to use "Freeze My MTC" as the subject.
Following the end of your requested freeze period, your monthly auto-renew will automatically be reactivated.
***USER AGREEMENT***

In order to assure your safety Supreme Training strongly recommends that you review the following User Agreement carefully before assuming any of the risks associated with personal training, gym membership or any other exercise related programs.
Supreme Training strongly recommends that you consult with your physician before beginning any exercise program.
Supreme Training is not a licensed medical care provider and represents that it has no expertise in diagnosing, examining, or treating medical conditions of any kind, or in determining the effect of any specific exercises as it relates to the varying medical circumstances.
You should understand that when participating in any exercise or exercise program, there is the possibility of physical injuring the muscle, muscle soreness and even death. If you engage in this exercise or any exercise program, you agree that you do so at your own risk. You have chosen Supreme Training as your fitness facilitator and have therefore assumed all risks of injury to yourself. You agree to release and discharge Supreme Training from any and all claims or causes of action, known or unknown, arising out of Supreme Training negligence.
All personal training services are final, non transferrable and non refundable.
